Colorectal Cancer Screening With Repeated Fecal Immunochemical Test Versus Sigmoidoscopy: Baseline Results From a Randomized Trial

چکیده
Background & AimsThe comparative effectiveness of sigmoidoscopy and fecal immunochemical testing (FIT) for colorectal cancer (CRC) screening is unknown.MethodsIndividuals aged 50–74 years living in Southeast Norway were randomly invited between 2012 2019 to either once-only flexible or FIT every second year. Colonoscopy was recommended after if any polyp ?10 mm, ?3 adenomas, advanced CRC found or, subsequent to, >15 ?g hemoglobin/g feces. Data this report obtained complete recruitment both groups included 2 full rounds part the third round. Outcome measures participation, neoplasia detection, adverse events. Age-standardized detection rates age-adjusted odds ratios (ORs) calculated.ResultsWe 139,291 individuals: 69,195 randomized 70,096 FIT. The participation rate 52% sigmoidoscopy, 58% first round, 68% 3 cumulative rounds. Compared similar round (0.25% vs 0.27%; OR, 0.92; 95% confidence interval [CI], 0.75–1.13) but higher (0.49% 1.87; CI, 1.54–2.27). Advanced adenoma lower compared at 1.4% 2.4% (OR, 0.57; 0.53–0.62) 2.7% 1.14; 1.05–1.23). There 33 (0.05%) serious events group 47 (0.07%) (P = .13).ConclusionsParticipation more adenomas detected with repeated sigmoidoscopy. risk perforation bleeding comparable. Implementation of population screening for colorectal cancer by repeated Fecal Immunochemical Test (FIT): third round
BACKGROUND Colorectal cancer (CRC) is the most common cancer in Europe with a mortality rate of almost 50%. The prognosis of patients is largely determined by the clinical and pathological stage at the time of diagnosis. Population screening has been shown to reduce CRC-related mortality rate. Most screening programs worldwide rely on fecal immunochemical testing (FIT). The effectiveness of a F...
متن کاملDiagnosis reliability of combined flexible sigmoidoscopy and fecal-immunochemical test in colorectal neoplasia screening
BACKGROUND Employing colonoscopy, the gold standard in colorectal cancer (CRC) diagnosis testing, for CRC screening presents a significant risk of complications. Alternative methods with a lower invasive-level and fewer risks are proposed in combination, though each with lower diagnosis performance when applied separately. The main objective of this cross-sectional pilot study was to evaluate t...
متن کاملSimilar fecal immunochemical test results in screening and referral colorectal cancer.
AIM To improve the interpretation of fecal immunochemical test (FIT) results in colorectal cancer (CRC) cases from screening and referral cohorts. METHODS In this comparative observational study, two prospective cohorts of CRC cases were compared. The first cohort was obtained from 10 322 average risk subjects invited for CRC screening with FIT, of which, only subjects with a positive FIT wer...
متن کاملSelecting a Cut-off for Colorectal Cancer Screening With a Fecal Immunochemical Test
OBJECTIVES Fecal immunochemical tests (FITs) for hemoglobin (Hb) are increasingly used for colorectal cancer (CRC) screening. However, cut-offs for defining test positivity are varying widely. We aimed to evaluate the impact of cut-off selection on key indicators of diagnostic performance in a true screening setting. METHODS We evaluated diagnostic performance of FOB Gold, a widely used quant...
متن کاملPerformance of the Fecal Immunochemical Test for Colorectal Cancer Screening Using Different Stool-Collection Devices: Preliminary Results from a Randomized Controlled Trial
Background/Aims We are in the process of conducting a randomized trial to determine whether compliance with the fecal immunochemical test (FIT) for colorectal cancer screening differs according to the stool-collection method. This study was an interim analysis of the performance of two stool-collection devices (sampling bottle vs conventional container). Methods In total, 1,701 individuals (a...
